Dead or Alive is the sixteenth and seventeenth episode of Season 4 of Dr. Quinn: Medicine Woman, the eighty-first second and eighty-third episode of the series, and the second split episode.
Ezra Leonard stops in town to deliver a speech on statehood. His son, Caleb, wanders away from the train station and is kidnapped by McBride. The ransom he demands is that Mr. Leonard promise to withdraw from the statehood movement and hand over the copy of the speech he plans on delivering in Washington, with the exchange spot being Cash Creek, a place where Sully is the only one in town who knows the location. As a result, Mr. Leonard requests that Sully lead the rescue team. When they get to Cash Creek, the exchange goes south when Mr. Leonard tries to take Walks Alone, Mr. McBride's wife, hostage. Mr. Leonard is hurt in the resulting fire fight, and Matthew takes advantage if it to go look for McBride, only to get abducted himself.
Meanwhile, Dr. Mike begins to experience some bleeding. Colleen is worried enough to wire for Dr. Bernard in Denver, When Dr. Cassidy comes instead due to Dr. Bernard being away, Dr. Mike refuses to take his advice, until she starts to hemorrhage, putting her entire pregnancy at risk.
After the exchange goes south, the rescue party begins to turn in to a mob and Sully goes off on his own to find McBride and rescue Matthew and Caleb. When Preston steps in a trap, and his wound gets infected, several members of the search party are surprised that Sully refuses to even consider just leaving Preston out there to die.
Meanwhile, Dr. Cassidy manages to stop Dr. Mike's hemorrhaging, and, with the help of Cloud Dancing, convinces her to stay home for a few days to rest before returning to work. Dr. Cassidy watches the clinic, which frustrates Dr. Mike as he treats Colleen as a servant, moves around the furniture, and tries to convince her to give up her practice and stay home. Also playing a role is Dr. Mike's lack of confidence in his judgement.
