Fear Itself is the twenty-fourth episode of Season 4 of Dr. Quinn: Medicine Woman, and the ninetieth episode of the series.

Isabelle Maynard, an artist, arrives in town and is embraced by the entire town. Preston even begins to have feelings for her, that is, until it gets out that she has leprosy. Everybody is too afraid of the name of the disease to even listen to Dr. Mike when she tries to tell them what has been learned about it in the last 1,800 years.

Meanwhile, Preston is planning a resort outside of town by the hot springs to draw in the wealthy who are willing to use the hot springs as a remedy for what ails them. He wants Dr. Mike to run the small clinic at the resort, but she and Sully question if that's the right thing to do.
